Our goal is to help you or your loved one remain safely at home. Our Home Care services range from companionship and light housekeeping to complex medical care and care for those on Hospice. As your needs change, our service expands to meet those needs. We also offer foot care and dental care in the privacy of your own home. Please look below at the many services we offer to help you or your loved one remain safely at home.
Homemaking/Companionship | Personal Care | Bath Visit | Medication Management | High Acuity Care | Foot Care | Dental Services
Homemaking/Companionship – This service includes:
- Light housekeeping
- Vacuuming
- Dusting
- Cleaning kitchen
- Cleaning bathrooms
- Laundry
- Changing linens
- Errands
- Shopping
- Meal planning and preparation
- Taking out the garbage
- Getting the mail
- Reading newspaper or books
- Going on outings or to doctor appointments
All services that include hands on care such as Personal Care, Bathing, Medication Management, etc., will be coordinated and supervised by a nurse.
Personal Care – This service includes all of the above plus:
- Bathing
- Dressing
- Grooming/Personal Hygiene
- Transfers - Assisting a person from bed to wheelchair, from wheelchair to shower bench, assistance standing etc.
- Incontinent Care
- Assistance with Ambulation
- Assistance with Physical Therapy exercises
- Coordination with Physicians, Home Health and Hospice
- Dementia Care
- Diabetic Care and Coordination
Bath Visit – This service includes:
- Assistance with a Shower, Bath or Bed Bath
- Changing bed linens
- Laundering bed linens and towels as time permits
- This visit will not last more than 1 ½ hours
Medication Management – This service includes:
If you or your loved one does not know what medication they are taking, why they are taking it, and when they are supposed to take it, then they will fall under medication management.
- Registered Nurse coordination with all physicians regarding medications. Our staff wants to make sure the correct medications are given at the right time, in the right dose and in the proper manner
- Monthly set up of medications in secondary containers
- Caregivers are trained to recognize and report signs and symptoms of drug side effects and interactions
- Ongoing coordination with doctors, home health and hospice
- Supervising caregivers to make sure that all medications are being administered correctly
- Coordination with family as needed and desired
High Acuity Care – This service includes:
High acuity services include Registered Nurse delegated tasks such as:
- Wound Care
- Administration of Insulin for Diabetics
- Feeding Tubes
- Care for those on Hospice. This includes all medications and personal care as needed
Foot Care – This service includes:
- Nursing assessment of the feet to look for fungus, corns, caluses, pedal circulation/pulse checks.
- Cutting of the toenails
- Filing down thick or nails with fungus
- Foot massage and lotion
- Coordination with physician as needed
Dental Services – This service includes:
- Initial Oral Exam
- Follow-up Oral Exam
- Adult Prophylaxis
- Full Mouth Debridement
- Periodontal Maintenance
- Root Planing and Scaling
- Fluoride Treatment
- Denture / Partial Cleaning
- Pre-Rinse
- Medical History Review
- Blood Pressure and Pulse
- Oral Cancer Exam
- Periodontal Exam
- Oral Hygiene Instructions
- Referral to Dentist
